Brief Answer:
may be atlanto axial dislocation

Detailed Answer:
hello
If she has RA for the past 30 years, and has also had a surgery, we are concerned about atlanto axial dislocation.
this is a complication that can come up in patients who have had RA for many years.
It requires an X ray or MRI for the cervical spine to rule this out, the treatment may be surgery or upgradation of her medication that she takes for RA.
