Brief Answer:
as explained in details.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Male/71 - on Warfarin - swollen top of foot - severe pain - foot elevated and applying ice - taking Tylenol ...
The short history is indicating that there might have been a minor or trivial trauma or some twisting or so that has caused the swelling.
I would advise your the following:
Urgent INR and related blood tests to decide the effectiveness of Warfarin.
Consult a Doctor to see for any other obvious reason like hematoma or cellulitis or fracture and to get the additional investigations accordingly.
Other tests of blood like CBC, Sugar, kidney and liver functions.
High resolution Ultrasound to see whether hematoma or only edema.
X-ray if the fracture is suspected.

I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication and give any additional history if you have.
