Normally there are depressions in tonsils and infection can get retained in these depressions.
If the symptoms of tonsillar infection are persisting for three months, these must be without fever and occasional pain.
Use of antiseptic gargles like diluted hexidine will help in keeping the infection under check. Only problem will be a 4 year child will not be very compliant for gargles. Still hot water sips, avoidance of junk and sticky foods will help.
Things should improve with these measures. If not please go for an ENT examination, and a throat swab should be taken to specifically prescribe an antibiotic depending upon the sensitivity report.
Post a course of specific antibiotic, the use of antiseptic gargles and dietary restrictions help.
If symptoms of tonsillitis persist and the episodes of acute tonsillitis frequently happen, tonsillectomy can be an option.
