I am sorry to hear that you are having difficulty controlling your diabetes. You had been on Januvia but side effects prevented your from taking the medication. Before I go any further I need to know what other diabetes medications you have tried. I have been taking Metformin 1000 2times a day for the last three yrs. Originally the dosage was Metformin 500 2times a day. I also take HCTZ 12.5, Atenolol 50mgs, Lisinopril 5mgs. and Livalo 4mgs. All these meds are once a day.

Thank you for returning to HCM with your information. If you were my patient I would add pioglitazone to your current treatment program. This is a insulin sensitizer so your body will become more sensitive to the insulin you already make. It should help control your sugars much better. The only reason I would not recommend this is if you have a history of heart failure as it can cause worsening of your heart failure. I do not think that insulin is needed or any other of the expensive new medication as pioglitazone is a very good controller of DM and you have not tried it yet
