Question: I am 7 and half month pregnant and diagnose with gestational diabetes. Currently I am taking Insulin 30/70 thrice a day, 6 units before breakfast, 6 units before lunch and 12 units before dinner. My sugar level is under control except after dinner. I have also control the diet as well as taking 30-40 minutes walk everyday and also do some household chores. Even though my sugar level after dinner is not under control. My gynecologist ask me to maintain up to 160 is also fine. but two hours after dinner it sometimes goes till 200 or above also. Kindly suggest the other precaution or dose or medicine I can take to bring my sugar level under control.
Brief Answer:
Need change of insulin plan.
Detailed Answer:
Dear XXXXXXX Hi! ideally GDM require 4 insulin injections a day. This 30/70 insulin is not the insulin for pregnancy diabetes. The targets for pregnancy are also tight . Fasting - 90 mg/dl, Post meal - 120mg/dl.
How many weeks of pregnancy are you going through, you have not mentioned.
I will suggest to you what I do with my GDM patients.
1. strict diet restriction
2. Insulin aspart (novorapid) 6/ 6/ 6 before breakfast, before lunch, before dinner
3. Insulin lantus 10 units bed time.
4. sugar monitoring 4 times a day - fasting, 2 hr post breakfast, 2 hr post lunch, 2 hr post dinner.
5. weekly follow up.
regards!
