What Causes Abdominal Pain, Low Platelet Count And Neck Stiffness?

Question: I was recently tested positive for EBV of greater than 5. For 2 1/2 yrs. i've been struggling with a domino effect of symptoms. Starting with Abdominal pain in upper left pain,pain in bottoms of feet and hands. Canker sores,neck stiffness and a lingering feeling of something off with my gut. I also tested positive ANA 1:1280. My platelets have been ranging between 45,000 -75,000 for 2 years. After many test like Bone Marrow biopsy,colonoscopy,upper scope,Hepatitis,Hiv,Lupus,Lymes,Ulcers,etc.... all came back negative. I also currently found out that i have either an allergy/intolerance to crab and corn. My questions are 1.) Could EBV be the possible cause of all the listed symptoms including low platelets and positive ana? 2.) My wife recently tested positive for EBV as well after she was went in for a skin issue called Pityriasis lichenoides chronica. I told her to get tested for EBV and they said yes she was positive. The spots on her have gotten worse. I'm guessing if i got a chronic case of EBV 2 1/2 yrs ago when my problems started and gave it to her, is it possible that she is getting these skin problems 1 1/2 yrs later? Why is it coming on so late from the first symptoms? 3.) As stated above,I tested neg for Hiv at 2 1/2 months with an hiv ab 1&2 and also viral load pcr rna test at 94 days that was non detected. Are those results conclusive? 4.) Is the at home Hiv Oraquick Hiv test that came back negative reliable results at 6 months and 1 year? 5.) Testing positive for EBV and Positive ANA ,is there any correlation between those two and HIV? 6.) If EBV isn't the cause of my problems than could my new discovered allergies to crab and corn be the cause? If my wife didn't have her recent skin problems than i would just think it's my food allergies but now i think it may be something together since she has been having a few health concerns as well. Brief Answer: Autoimmune disease is a strong possibility Detailed Answer: Hi there, Thanks for your queries. The answers to your queries are as follows :- 1. (a) EBV is apparently neither responsible for your symptoms, nor low Platelet count. (b) However, there is evidence that infection with the EBV is associated with a higher risk of certain autoimmune diseases, especially Dermatomyositis, systemic lupus Erythematosus, rheumatoid arthritis, Sjögren's syndrome, mixed connective tissue disease (MCTD) and multiple sclerosis. (c) High ANA titers indicate presence of an autoimmune disease. Your symptoms, though inadequately described, are suggestive of an autoimmune disease, most likely MCTD. Consult a rheumatologist, who will order for more specific antibodies' test to pinpoint the disorder and advise you the specific treatment. 2. Your wife's skin condition is not likely due to EBV. It may either have a different etiology or, as in your case, an autoimmune disease (SLE or Dermatomyositis). 3. The HIV tests done, which are very sensitive and specific, have ruled out HIV conclusively. 4. Home HIV test- if negative at 6 and 12 months- rules out HIV. 5. There is no correlation between EBV, ANA and HIV 6. Newly detected allergies are unlikely to cause your symptoms particularly gut, provided you have been avoiding crabs and corn and their products. Consult your doctor and apprise him of my opinion. I have seen a couple rheumatologist that did run extensive test like systemic lupus Erythematosus, rheumatoid arthritis, Sjögren's syndrome, mixed connective tissue disease (MCTD) and multiple sclerosis,Lymes.. after discovering a positive ANA 1:1280 but all came back negative. My vitamin D was also low at 22.3. All at the same time a have severe foot pain on the bottom arches of my feet. So for the past 2 1/2 yrs. i've struggle for answers to the cause while i live in pain.My discovery of EBV and the allergies to crab and corn came to light per my request to be tested for them since we have been testing for everything else and still don't know that cause of the symptoms. My health was perfect with never any issue until what seems to be overnight starting with severe abdominal pain that woke me up out of my sleep. Within 1 month i had 3 very painful abdominal attacks,the 2nd attack including diarrhea and vomiting. From that point on it has been all downhill for 2 1/2 yrs. The pain has gone away in my gut but at times still feels like something is a bit off.. my question is 1.) If i had EBV at the time or even the crab/corn allergy could this cause that kind of pain in the upper belly just to the left of the belly button? 2.) my platelets went as love as 45,000 which historically from past records my platelets were always in the normal range, could these be causes of that as well? 10 months later is when i started to develop a very stiff neck that has been in pain for over 1 1/2 yrs now. It seems that it has become a domino effect when i felt like i was still in my 20's and just like that it is all come crashing down. Could EBV or Food Alllergies cause the sudden drop in Platelets? 3.) The only positive sign i've had recently is this past Dec. when i found out about the allergies and tried to avoid Corn and crab the best i could. My monthly CBC showed my platelets went up for the first time from an average of 70,000 a month to 140,000 for the first time in the 2 1/2 years.. I was starting to believe that this could've been the cause of the problems but with my wives complications with her PLC skin problem, i'm starting to feel that maybe a chronic case of EBV could be the cause of both our problems. I'm not trying to diagnose myself but living an pain is sometimes unbearable and i shouldn't have to go thru life like this any more. Please help and thank you for your time

Brief Answer: Need to explore other possibilities Detailed Answer: Hi, Firstly, sorry for the delay in response, due to different Time Zones. I can fully understand your concerns, frustration and helplessness. 1. We have run out of options as all tests for autoimmune diseases have been carried out by rheumatologist and found to be negative. However, Polymyositis has not been ruled out by him. Also, your symptoms indicate possibility of rheumatoid arthritis, for which the Anti-CCP test (diagnostic test) has apparently not been done. 2. Chronic MILD EBV Infection (in view of a persistently low platelet counts)- though remote- is a possibility which cannot be wished away. Incidentally, the symptoms of chronic active EBV infection include fever, liver dysfunction, enlarged spleen (heaviness or lump felt in left upper abdomen), enlarged lymph nodes (lumps in armpits, neck and other regions), anemia, rash, oral ulcers, to name a few. However, almost 95% of the US population get EBV infection by around 40 years of age. Hence, a positive EBV test do not necessarily indicate chronic EBV infection. PLEASE UPLOAD YOUR EBV TEST REPORTS (old and recent) for my perusal. A turnaround in the Platelet counts is a good sign, which apparently signifies gradual improvement. Test for Platelet counts at regular intervals in future, would provide us with the prognosis. 3. Allergy to corn and crabs commonly begins/occurs in your age group. It has nothing to do with either, EBV infection or, low platelet count. Further, though abdominal pain is a symptom found in EBV infection, it is apparently due to crab/corn allergies (abdominal pain with diarrhoea and vomiting that you had sometime back). 4. Therefore, since EBV-induced auto immune disease have been ruled out, other causes- as listed below- of your problems have to looked into/considered. * Apart from autoimmune disease, EBV infection can also cause Chronic fatigue syndrome, the symptoms of which are severe chronic fatigue , impaired memory or concentration, muscles pain (in your case, neck muscles), sore throat, joints-pain (hands, shoulder as in your case), headaches, disturbed sleep. Incidentally, do you have tender lymph nodes in neck or armpits? * Fibromyalgia, the symptoms of which are similar to those of Chronic Fatigue Syndrome. In addition, there may also be anxiety, depression, and disturbances in bowel function. * Planter fasciitis, in which there is inflammation of the protective sheath in the soles of feet, which causes persistent pain in the soles, particularly during sleep and prolonged rest. * Stress fractures of the feet's bones due to osteoporosis, following vitamin D deficiency. 5. Lastly, effective treatment will depend on the specific diagnosis, by excluding the possible above-mentioned causes. This notwithstanding, I feel that COMBINATION of a course of :- - anti-inflammatory drugs, - an anti-depressant drug (belonging to a Tricyclic group), such as, amitriptyline along with Fluoxetine; or, Duloxetine - Vitamin D3 (Calcitriol) 0.5mcg with at least 1000mg of calcium should give you substantial relief. However, your doctor is the BEST JUDGE. You may therefore, consult your doctor again and apprise him of the possibilities given by me. He is expected to actively consider my opinion and proceed to short-list the exact cause by way of exclusion. Take care
