The white spot under the tongue can be because of the frictional keratosis secondary to chronic irritation by the sharp teeth. The other possible cause can be leukoplakia(if you the habit of tobacco chewing) or fungal infections like candidiasis. Nothing to to be panic, consult an oral physician and get it evaluated rather than oral surgeon. It cannot be a scar unless there previous history of surgery in that area.
The white spots and pain in the neck and shoulder are not related. For this issue, you can consult an orthopedician and take an opinion.

If it is because of frictional keratosis or leukoplakia nothing to worry, it won't harm you. If it is causing some discomfort then you can get it surgically removed. If it is a candidiasis then it can be treated with a course of anti fungal agents.
