Question: I am an 86+ year old female. Almost 6 years ago I flew to XXXXXXX on Southwest Airline. Within 3 days I began having Cough, sore throat, ear ache and lost my hearing in my right ear for about 6 months. It finally returned to normal. I have this awful taste in my mouth, all the time. I lost 20 lbs. because, I didn't feel like eating. I was nauseated, and had a very dry mouth at night. My tongue stuck to the roof of my mouth and felt like cellophane. I would try to gently move my tongue to moist my mouth for me to feel safe to move my tongue. During the day my mouth is so moist I drip if I bend over. I couldn't sleep unless my head was raised.
I went to my primary care Doctor who referred me to a dozen or more specialist, they all treated me for GERD including laparoscopic hiatal hernia repair with Nissen---fundoplication Which didn't help (at all). I have had all kinds of tests Barium Swallows, throat test, hearing test and other test for GERD. I take Nexium--because they told me to. I didn't have heart burn before . The Doctors have all given up on a Diagnosis. The last Doctor (an internist) told me he didn't know what to do.

I have ask every Doctor I have seen in the last 5 1/2 years if I could have gotten something on that plane, and no one paid any attention. The last Doctor--also told me he didn't know what to do. As I was walking out the door, when I mentioned the plane, he gave a prescription for Nystatin Oral Susp 100000/ML I swished and swallowed 5ML BY mouth three times a day for 2 weeks. Within 2 days most of my symptoms disappeared and in a few days I was symptom free. This lasted for about a week and then they returned. I had an appointment with another Doctor who is now treating for very high white blood cell count. I mention to him that the Nystatin helped my symptoms ( my white blood cell count had come down a little, since my last appointment) He immediately ordered a refill for the Nystatin for 2 weeks. I have finished the 2 weeks and the symptoms are coming back. They have not done a throat culture and what are your thoughts about what we might do next.
I know this is long and complicated, but I am still suffering . It will be 6 years at Thanksgiving. Can you help me? It has taken me a long time to type this, as I haven't type much since high school. I also do not use the computer often.

Hello,

summarizing your complaints... you've got a strange sensation on your tongue, nausea and your mouth full of fluid most of the time particularly when sleeping. Well, it does sound like GERD, but surgical correction was supposed to - at least - reduce the symptoms. Esophagitis is another consideration and since you've seen improvement with antifungals (nustatin) Candida is a potential cause. I can't connect the airplane incident with your current symptoms.

Candida causes characteristic lesions on the mucosal surfaces (they look like if you've spilled milk over the mucosa and there may be some redness too). The esophageal lesions can only be visualized with an upper gastrointestinal endoscopy.

If you've done any tests you'd better upload them because it may help me to exclude various disorders. Also please describe your symptoms and history in more detail. For example:
- do you use any drugs (not only for this symptom)?
- do you have any other medical history?
- do you have heartburn or feel like choking after swallowing?
- do you have fluid in your mouth if your stomach is empty and you're not lying down?

I'll be glad to continue this discussion when you upload the documents.

I will try to answer your questions. My friends daughter came over and installed a flash drive with numerous pages of reports, but it is not transferring the material. She will have to find out what is wrong and try again

For years my Doctors have ask me to write down a list of things I need to discuss, so I do that with every appt.

I had very bad heartburn with all 7 pregnancies , but had not had any problems since my last baby 45 years ago.
My tongue gets very dry and red. It burns so much at times, I don't talk because it is so painful. The taste is so horrible, I try to brush it off and that hurts, but gets rid of most of the taste. I worked as a Dental Assistant for 25 years, so brushing my tongue was routine. My mouth is very moist all day, but nights it gets very dry and my tongue sticks to the roof of my mouth. It feels like cellophane and I try to make saliva before I try to move it. I am told I do not snore nor mouth breathe. I bought an adjustable bed, because I could not sleep lying down. I do not get lesions on my tongue, I get very hoarse at time, and get this lump on the very back of the roof of my mouth every morning, which gags me and I have to cough until it come loose. I cough when I eat and at other times of day or night.
We are trying to upload numerous pages of reports and results and can't do it until I find someone who can help me. In the mean time, here is the list of medications I take Nexium, 40 mg 1 daily, High blood pressure -- Norvasc10 mg 1 day, Labetalol 100 mg 1 day, Diovan 160/25, & 81 mg aspirin 1 day. I have had high blood pressure since I was pregnant with my last child-50 years ago. All my family members die from strokes or heart attacks at age 90+ so I plan on living another 5-10 years--the good "LORD" willing. I would like to feel better. AZO for UTI PRN. I take Hydrocodone/Acetaminophen 10-325 following a procedure to burn sciatic nerve in my right hip. Premarin cream 2xweek, Zertec for sinus infections, which I haven't needed since a balloon procedure early this year.
No I don't have what I might call fluid in my mouth. I have a lot of saliva, but no taste from it and it is clear. I have no problem swallowing. I drink a lot of water. I
I am a very active person, I am now 5' 2 1/2" tall (was 5'4") 120 lbs. I am active in my church groups, I belong to Operation XXXXXXX which collects used and new toys for the less fortunate. I wash and repair all stuffed animals we receive. I do my own yard work (except mow) and keep my 4 bedroom house clean. I go out with friends and did water aerobics until recently. I just get too cold in the pool. My temperature is around 94 and has been all my life. I am a very healthy person, never smoked, drink very little (I prefer ice tea) I have not had colds or flu since winter 1970. I am allergic to egg yolk, so can't take the shot. Mosquitos do not bite me, they land on me-but don't bite. My Dad always told us "If you aren't dying, don't complain" But I am so tired feeling bad, I hope we can figure this out. I have no follow up appt. except with the Dr. working on my WBC.

Everything would have fitted perfectly for reflux but taking nexium and having no heartburn is very much against it. Perhaps it's an inflammation of some sort of the tongue (geographic glossitis? Vitamin deficiency?).

The tests would be very helpful. I particularly need to see your vitamin B12, iron, ferritin and folic acid levels. A good quality picture of your tongue (with good lighting and well focused) may help. If you have the reports as files you can upload them either through this website or elsewhere and send me the link.

I have tried several times to send the reports via e-mail, and so far they will not send. Tomorrow A friend will send them through her computer. She has them on a flash drive. As I said earlier, I do not know anything about computers, and at 86 I am not very interested in learning. But I will get them to you. Thanks for your patience. I had a repair man today, he says there is some problem with my computer.

Hello again,
XXXXXXX sent me the reports and I've taken a look at them. Although I didn't see the tests I've asked for (at least not recent ones), I believe that your problem is probably the reflux issue. For a patient like you I would have asked for a recent upper endoscopy to check the esophagus and serum indices like vitamin B12, ferritin and folic acid. Comparing older blood counts with current ones sometimes provides indirect clues regarding B12 and folic acid deficiencies. I've checked your blood counts and it seems that you probably don't have any deficiencies but doing the aforementioned tests is the only certain way to find out.

Since you've found relief in antifungal treatment taking a proper course of fluconazole would probably benefit you. The tablets are usually the first option and the dosage depends on body weight (3-6mg per Kilogram). The total duration of treatment is 14-21 days or at least 1-2 weeks after cessation of the symptoms.

So in conclusion, I believe that the fungus issue has to be clarified with an endoscopy and some blood tests (mainly for deficiencies) are required to get a better idea of your condition. Antifungal treatment will help if the endoscopy is positive. Drugs against acidity are required to reduce stomach acidity. Omeprazole and other prazoles (proton pump inhibitors) are fine options for that.

I really hope you'll find some relief eventually!

I am scheduled for an endoscopy on MON. Sept. 18th. As for the B12 and folic acid-- I have taken B100 2 X a week for 30 years. I am due to have a complete lab workup Oct. 17. I will ask the Dr. for the specific lab. test you requested, when I see him on Monday. Sorry I don't have the up to date results you need. The symptoms are not as severe as before the Nystatin, and they all disappeared while taking it, but some are slowly coming back. Thanks for your help.
