just 75 and after a he had an orgasm with me he was very breathless almost speechless. he has some circulation problems and swollen ankles but insists he is fit

Considering his past medical history (circulation problems, swollen ankles), I would recommend performing some tests to investigate for the possible underlying causes:

- a resting ECG and a cardiac ultrasound to examine his heart function and structure
- a Doppler ultrasound of the cervical arteries to examine the blood flow to the brain and exclude possible narrowing
- some other blood lab tests (complete blood count, kidney and liver function tests, blood electrolytes, fasting glucose).

A brain MRI would help exclude possible small strokes.

He may need to start anti-platelet agent or anti-coagulation therapy (in case of heart failure) in order to prevent possible strokes in the future.
