Hello. Thank you for writing to us
I have gone through your query and I have noted down your concerns
It would be really helpful and easy for me to understand if you can upload a few representative images of the condition affecting armpits, thighs and adjacent regions.
Though from your description it appears to me that we are possibly dealing with fungal infection Or intertrigo, however I would be able to guide you better if you can please upload an Image.
You may upload an Image either at "Reports Section" Or You may mail me at YYYY@YYYY with the subject line as Attention:Dr.Kakkar.
Your second concern is perianal itching.
Perianal itching can be due to various reasons like due to other associated conditions e.g piles/ tags etc Or due to intertrigo/ fungal infection Or irritant dermatitis.
Any possible underlying cause e.g piles/ tags etc need to be identified and managed appropriately.
Other than that If I was the treating doctor I would suggest that you use an antifungal+steroid combination cream for a few days initially e.g clotrimazole 1% cream + beclomethasone dipropionate 0.025% cream, twice daily. Thereafter you may use an antifungal dusting powder regularly in folds between your butt cheeks to keep the area dry thereby preventing the fungus from growing again.
Once daily tablet cetrizine or tablet levocetrizine for a few days in evening will also help resolve itching

Stretch Marks in axilla; Use plain antifungal cream in axilla
Detailed Answer:
Thank you. I have reviewed the images.
These bright red, linear marks in axillary region are actually Stretch Marks, also known as Striae and are probably due to Steroid content of topical creams that you may have been using over this area.
Potent topical steroids can cause skin thinning, atrophy and stretch marks if they are used unsupervised over a long period of time. Stretch Marks are irreversible.
Regarding itching and scaling in axilla, that is most probably due to fungal infection. I suggest you to use a plain topical antifungal e.g either Eberconazole (Ebernet cream) Or Miconazole (Zole cream), twice daily for a 2-4 weeks.
These are steroid free prescription drugs and I suggest you to use these only under supervision of a dermatologist in your area.
A tablet of cetrizine 10 mg once daily will provide you symptomatic relief from itching.

You are welcome
