Question: What could be the cause of a burning sensation occasionally under nail beds of the feet, usually when lying down?

Since how long you are suffering from this problem?
Any history of diabetes or hypertension?
Any history of smoking?
Any history of trauma to foot?
Any changes in overlying skin?

Burning sensation do seen in few conditions where there will be reduction in blood flow to foot or some skin conditions do result in such a symptom. Examination for peripheral pulses and skin changes over foot helps to rule out few conditions and investigations to assess for blood flow is required depending on your examination findings.

Thank you. Fortunately I have none of these conditions, and there is no discoloration. The sensation only appears when lying down occasionally. I have noticed it for a few months.

Hi, thanks for follow up.

Good that you are not suffering from any co morbid conditions. Burning sensation most probably from peripheral neuropathy, which can be confirmed with tuning fork test and vitamin B complex supplement do help in this condition, injectable form are better than oral medications. Do get color Doppler of lower leg to assess for vascular sufficiency.
