Question: 20 year old developed neck pain 2 weeks ago. Large swollen gland possibly at base of skull, swollen lymph in the front of the chest, back shoulders, down spine. Feeling of something under left ribs moving collar bone up. Feeling of esophagus not working right as if it is misplaced. Legs and arms turn red on and off and appear to be retaining fluid. Left side does so more than the right. Lymph nodes under arms and groin are not swollen. Chest and neck xrays did not show any injury. Pain in neck radiating from back of skull to eyes is intense. vision blurry, Dr. prescribed steroids, muscle relaxers, almost done, some swelling in body improved but it comes and goes. periods of difficulty breathing but will resolve and start again. high heart rate for several weeks and blood pressure, weight loss of 10 lbs within about 2-3 weeks. Labs ordered but not stat, patient unable to remain up and about for any period of time due to extreme neck pain, radiating to head. No history of health disorders except for low vitamin B 12. No history of sexually transmitted disease.

I forgot, several days ago she felt a triangular shaped small hard object under the skin in her collar bone. She could move it around and it shot under her collar bone. It did not hurt.

ON the previous case sent in just now, additional symptom includes numbness on various body parts, including throat and ears

Hi XXXXX
Welcome to HEALTHCARE MAGIC..

How was your day?

Hope this message finds you in good health.

I have gone through your query in detail and understand your concern.

from the history it seems to be a multi system hormonal imbalance issue causing fluid retention inside the body causing variations in heart rate and blood pressure. lymph enlargement can be caused due to any abdominal or chest infection.

neck pain though has many possibilities..

POSSIBILITY 1. SOFT TISSUE INFLAMMATION- there may be no bony involvement, only the soft tissues around the region might be inflammed that may be causing the pain

POSSIBILITY 2. NEUROPATHY- there may be a partial or complete inflammation of the nerves in that region that may be the root cause of such kind of pain.

POSSIBILITY 3. DISC PROLAPSE-there may be a compressed a nerve in the spine somewhere that might be causing radiating pain in those regions.Possibility of a disc prolapse should be kept in mind.

the blurry vision may be due to involvement of a cranial nerve as well.

she needs to get a multi system approach from a physician doctor and rare diseases and syndromes should be ruled out as she is too young to have so many symptoms.

I suggest you get an Xray as well as an MRI done to find out the exact cause.

Physiotherapy,analgesics,anti-inflammatory drugs should help till then.
Do get back to me after ur reports are ready.

she should eventually get back to normal in coming few days.Nothing to worry about.

This morning the lymph nodes at base of skull is hard as a rock. The smaller ones under her jaw bone, chest and back are hard to. She has fluid in her face, chest abdomen, worse on the leftop side. She did have x-rays which showed nothing.

Lymph nodes, small and hard in groin, under arms, back, chest. Worst pain is still the large one at the base of skull. Labs done were to check for STD's, hepatitis, ESR, LDH and CBS. All normal with the exception of white blood count being slightly elevated. Today, all joints inflamed, burning in fingers and legs, heart rate over a 100 resting at all times of the day. fluid came out behind left knee. There are knots behind both knees. She is going in for cultures tomorrow on blood, shot of antibiotic and oral antibiotic. Frightened at weight loss and condition worsening. She has one more steroid pill. Do you have any clue as to what this could be?

Hi XXXXX

Welcome back
enlarged lymph nodes in so many areas of the body indicate a generalised blood infection which is spreading all over the body. increased wbcs also indicate that .

do not give steroids or the infection will worsen.

she needs to be admitted in hospital and kept under medical supervision.. give her iv fluids, start on iv higher antibiotics, diuretics - for draining out excess fluid retention in her body.

I hope your question has been answered.
