Brief Answer:
Visit a Proctologist for physical examination

Detailed Answer:
Hello! Thanks for putting your query in HCM. I am a Gastroenterologist (DM).
Dull pain in anus can be due to many reasons like:
1. Hemorrhoids
2. Anal fissure
3. Abscess
4. Proctitis
5. Inflammatory bowel disease or any infection or malignancy
Since colonoscopy was normal 7 years ago, and pain is anal area only, I will suggest you to visit a Proctologist for physical examination and further management
I hope I have answered your query and this will help you. If you have any further query I will be happy to answer that too. Remain in touch and get-well soon.

Brief Answer:
You should get repeat colonoscopy at the age of 50 years

Detailed Answer:
Welcome back. Your PSA level are normal so there is nothing to worry about that.
You can have repeat colonoscopy after 9years from now. Presently you may simply get proctoscopy to see the cause for anal pain.
Usually colon cancer is found at or above the age of 50 years or earlier if there is predisposing risk factor like inflammaotory bowel disease, polyp in your previous colonoscopy, personal history of any other cancer, family history of colorectal cancer. However none is there or you havent mentioned. So the risk of colorectalcancer is very low or not there. However I will still suggest you to go for proctoscopy examination. I hope I have answered your query and this will help you. If you have any further query I will be happy to answer that too. Remain in touch and get-well soon.
