What Causes Esophagitis And Gastritis Along With Tenosynovitis?
Question: Hi i've was admitted to the hospital 2 months ago for Viral AGE with AKI and was also diagnosed with Antral Gastritis, Z line at 40cm Esophagitis LA Grade A and D1 Duodenitis. I have a drinking problem (since 1 was 22yrs old ) , 8 - 16 cans of beer daily or 75cl Vodka (or at least 5-6 days a week). I always get loose stools (6-7 yrs now) or flat stools, sometimes very dark, sometimes black sometimes normal color (get worst after drinking). It is sticky and leave yellow stains in the bowl. Ive been prescribed Pantropazole but still persists.
I just stopped drinking 3 days back when i was diagnosed with Tenosynovitis. It happened when after I stretch my fingers. The doctor said that it is infectious type because of the Kanavel Sign. It only hurt at the joint below my finger (metacarpophalangeal joint), the upper parts of the finger is not painful. Doc prescribed Cloxacillin and now condition improved a lot, though still mild pain at metacarpophalangeal joint if pressed..
The doctor also said that with my history of esophagitis, gastritis and now tenosynovitis, he suspects that i have an underlying issue and that my immune system is very weak. He asked if I am diabetic which i am not (just found out i am Pre-Diabetic). I have yet to make a follow up on this. But this is getting me worried cause I had unprotected oral sex 7 month ago. What underlying cause can I have? Weakened Immune System can be a lot of things and most are usually bad and im very worried. Can you shed some light?

Brief Answer:
Pancreas
Detailed Answer:
Sorry to hear about your medical challenges.
When I see someone with your type of complaints in my practice, I order the following tests in addition to a thorough physical examination:
CBC
Electrolytes
Calcium
Liver function tests (SGOT / SGPT / Albumin / Bilirubin / Globulin / Alkaline phosphatase / GGT )
Kidney functions tests ( Creatinine, Bicarbonate)
HbA1c
Stool for fat
CT scan of the abdomen with focus on pancreas
The purpose of the above tests is to look for potential reasons for your symptoms.
It is possible you may have Chronic pancreatitis from alcohol. This can cause the bowel problems you are experiencing and raise blood glucose levels too.

Hi Dr. Shehzad thank you for your swift reply. They have done all the tests when I was admitted at the hospital. Also did CT scan etc. The diagnosis is Antral Gastritis, Z line at 40cm Esophagitis LA Grade A and D1 Duodenitis. Pancreas appears normal, Liver Appears Normal. I will follow up on this issue and take your advise to check on pancreatitis. However you didnt answer all of my question. The following part is making me worried:
The doctor also said that with my history of esophagitis, gastritis and now tenosynovitis, he suspects that i have an underlying issue and that my immune system is very weak. (Does my symtoms and illnesses suggest Cancer, HIV or anything else? Cause these are the possible outcome when i entered esophagitis, gastritis and tenosynovitis in XXXXXXX symptom checker. What are the chances? Do I need to test for these?)

Brief Answer:
Follow up
Detailed Answer:
I have noted your comments in detail.
Theoretically anything is possible and so it is not worthwhile worrying about it. It would be more prudent to simply get checked for it.
I would suggest a comprehensive evaluation by an Internal medicine doctor who must have the opportunity to examine you in detail. Then it can be determined if testing for these conditions is warranted or not.
