
What Causes Fluctuating BP?

do i have hypertension. i am 30 years old.
I had taken Locar-50 one month back for one week and then stopped taking it. Later after one week i went to heart specialist and he took ECG and Renal Doppler test and told everything is OK and i dont need to take medicine. Today when i went to a physician who earlier recommended me Locar, again he measured my BP as 140/96. i am very worried. shall i start taking medicine again. i have a automatic BP monitoring machine also which gives reading 130/90 and sometimes 124/86 also.

Dear Sir
1. The readings which are above the cut-off i.e. 140/90 mmHg should come persistently for weeks (8-10 readings in 3-4 weeks) before we could label you hypertensive or you should have evidence of effect of high blood pressure on your end organs or you must have had one episode of very high BP with clear cut symptoms due to BP. If you have any one of these , only then you should be labelled as having high BP.
2. Moreover, While taking BP it should be done under ideal circumstances. These include No coffee/tea/exercise/smoking 1 hour prior to measurement, No stress or physical pain like headache, preferably at same time of day, relaxed & sitting at least 5 minutes prior to measurement.
3. In cases where there are some of the readings are higher than cut-off, 24 hour ambulatory BP monitoring without drugs is recommended and very useful.
4. We do look for end-organ damage like LVH on ECHO or ECG, Renal dysfunction, urine examination and clinical examination.
5. I would recommended 24 hour ambulatory BP monitoring to most of my patients with similar profile if I do not find evidence of end organ damage.

Only once my BP was measured as 170/110 but I was not having any chest pain or headache. I was under heavy stress as one of my relative got paralysed. She was 64 years old. Now I have got a thing I my mind that I can also get the same due to one time high BP. Please guide me. A lot of people have consoled me but this thing is just not leaving my mind.

A onetime reactionary rise in BP in the past can not cause stroke now.
As I told you earlier, if no end organ damage is found on evaluation, the BP levels mentioned by you do not warrant any drug therapy now. However you must continue with life style modifications. These include low salt diet, a diet rich in nuts, legumes, vegetables, fruits and low in dairy products and saturated fats. Avoid alcohol and smoking. Maintain optimal weight with body mass index of 23. Daily meditation/ relaxation exercises. Daily brisk walk under supervision.
