What Causes Heavy Bleeding During Periods?
I am XXXX from XXXXXXX I have been recently suffering from heavy bleeding during periods, although periods are regular and there is no bleeding between the periods. USG report showed fibroids measuring 3.7*2.6 cms and 3.4 * 2.4 cms (anterior wall), 3.0 * 2.6 cms (left lateral wall) and 3.0 * 2.5 cms (post wall).
I am getting married next month. I would like to know if HIFU can be done considering my past history of myomectomy and whether it is safe for my future fertility?
Greatly appreciate your help in this matter.Thank you
PLEASE PROVIDE FEW MORE DETAILS, XXXX
Detailed Answer:
Dear XXXX
Thank you for trusting me with your health concern.
Could you please provide me with a few more details, so that I can offer a better opinion ?
1. I need to have the hysteroscopy and laparoscopy report of the procedure that was done 4 years back. Can you upload that ?
2. Can you upload the latest USG report that you have quoted ?
3. Again, in the 4 year period between previous myomectomy, and current scan showing fibroids, were you never symptomatic for fibroids ? Was it a fibroid free interval, and are these new growths this time round, or residual from previous surgery ?
4. What is your height and weight please ?
Looking forward to hearing from you with these details.
Take care.
Dr. Aarti Abraham
Thank you so much for ur quick reply.
Regarding some queries that u had asked me about:
1) Unfortunately, I dont have my hysteroscopic and laparoscopic reports with me right now as I am working in a different place.
2) I have uploaded the latest USG report with the pictures
3) Ma'am, I just want to make a small correction here. My previous surgeries were done 3 yrs ago in XXXXXXX 2011 and not 4 yrs ago as I had written by mistake. The doc who did the myomectomy said that she has cleared all of the fibroids as a result of which I was completely normal with absolutely no symptoms. Although I never took an USG scan after the surgeries until now, I believe these fibroids have reappeared after the surgeries. I did not have any pelvic pain at all like due to endometriosis. My symptoms started just for the past 2 menstrual cycles with the last cycle being the worst.
4) My height is 5 ft 4 inches and my weight is 46.5 kgs. My hemoglobin is 8.4 (it was 9.2 a yr ago)
Thank you Ma'am

HIFU CAN BE DONE
Detailed Answer:
Hello XXXX
Thank you for the updates.
The prior documents would have helped me in comparison and evaluation.
That said, HIFU can be tried in your case.
THere are 3 discrete lesions, and they are within the size range stipulated for good success rates in HIFU.
Regarding future fertility, HIFU does not impair surrounding tissues and targets fibroids only.
In case it fails or fibroids recur, then surgery is again mandated.
There is no reason why your fertility would be impaired.
One of the fibroids is impinging on the endometrial cavity, and that could lead to early pregnancy loss, but not infertility per se.
The best advice for you is to get married, have the HIFU / surgery and then plan a baby straightaway, without waiting for too long, since fibroids are tending to recur, particularly in your case.
Preoperatively, progesterone or GnRH analogues could be given to shrink the fibroids, and make them more amenable to HIFU, please discuss this option too with your doctor.
WIsh you best of luck always.
Take care.
Thank you so much for giving me clear clarity regarding my health issue.
Greatly appreciate it.
I just had one more question. Are you aware of any good hospitals/doctors in XXXXXXX who are experienced in HIFU?
Thank you
