I have attached photos of his upper and lower gums. The photos show they are swollen but the red spots are not as visible. There are no rashes on his hands, feet or neck.
Yes, his lymph nodes are swollen. There is no family history of any medical problems.
He still does not have a cough but is on the fifth day of a 102 fever. He still doesn't want to eat and says it's because his gums are sore, not because he feels sick.

Thanks for the images. I would recommend you to visit a pediatrician for complete clinical evaluation of enlarged lymph nodes and hepatosplemomegaly. Tests including monospot for Infectious mononucleosis, CBC, CRP and blood culture will be useful. Start him on vitamin B complex and Vitamin C supplement along with paracetamol for fever at present.
