Brief Answer:
This intermittent pain could be due to acute gastritis and duodenitis.
Detailed Answer:
Hi,
Welcome to health care magic,
This intermittent pain over left upper and middle stomach is likely to be due to acute gastritis and duodenitis here as you are also being treated for GERD,dyspepsia and IBS.
It was also seen on your upper gastrointestinal endoscopy report.
Here,I would like to know about the medications that you are taking at present.
The symptoms can be improved with some dietary precautions apart from medications.
Kindly get back to me with current medications.
Regards,

Brief Answer:
Does not seem to be pancreatitis.
Detailed Answer:
Hi,
Thanks for the follow-up.
Here, this intermittent pain does not seem to be due to inflammation of pancreas.
However, it could be related to IBS also for which you are already taking treatment like VSL#3 (probiotics bifidobactrium and lactobacillus),colospa retard(mebeverine) and immodium sos which is a antidiarrheal drug.
Here,I would suggest you to continue this treatment along with antibiotics (ofloxacin with ornidazole,if you had not taken earlier for gastroenteritis) and antacids (pantoprazole or rabeprazole with syrup digene 2 tsf twice daily) to get rid off this intermittent pain.
You can discuss with your treating doctor who may give prescription of this drugs.
Now,apart from medications you should also take dietary precautions.
1) eat plain foods like boiled rice and green leafy vegetables without adding spices or oil.
It will help to neutralize digestive acids and also they are easily digestible means there will be no need of extra stomach acid.
2) avoid oily, spicy and fast foods
3) avoid eating full stomach at a time rather have small meals frequently.
4) drink coconut water will help to reduce burning or pain.
5) eat lots of fruits like banana and watermelon.
6) avoid excess tea and coffee.
7) eat or drink buttermilk and yogurt twice daily.
Hope this detailed information will help you.
Regards,

Thanks a lot for the quick response Dr.only reason I thought pancreas is because of pain radiating to back too.didnt know that IBS can cause this too.also Dr how often should I do colonoscopy and endoscopy
Brief Answer:
Endoscopy and colonoscopy can be repeated after 3 to 6 months.
Detailed Answer:
Hi XXXXXXX
Thanks for the follow-up.
Here, there is rare chances of having pancreatitis because in that one would have more symptoms from just a intermittent pain and also it would be severe.
So,need not worry about pancreatitis.
As there is no such time limit for follow-up endoscopy and colonoscopy but it may be repeated after 3 to 6 months of regular medications and dietary precautions or as per your treating doctor's opinion.
Hope this will help you.
