Question: Hi I've been had my implanon implant in for almost 3 months now and for the last month and a bit I have had very irregular bleeding. My periods are much heavier than usual and the blood has been very bright red, they have also been lasting much longer. I also get heavy bleeding straight after intercourse. I was just wondering if this is normal? Thanks

Brief Answer:
implanon side effect---irregular bleeding

Detailed Answer:
Hello,
Thanks for the query to HCM,
Implanon is a progesterone implant which acts as a contraceptive by increasing the progesterone levels in the blood and hence prevents LH surge and ovulation thereafter. Initially after implant, it takes about 3-5 months in most women who have been using it for the first time to adjust to change in hormone levels and hormonal balance is generally established in few months. In the meantime the woman experiences irregular inter menstrual bleeding in form of spotting to scanty reddish brown bleed and often heavier bleeds.
This generally subsides by 4-5 months when slowly amenorrhoea is achieved till next 3 years once progesterone levels stabilise.
Another important reason after the implanon implant for irregular bleeding could be continued stress during the month be it physical , emotional , psychological, or even professional which can again cause an imbalance in new progesterone levels causing irregular shedding of endometrium.
So, there are two ways of tackling the bleed in this period, one to use antifibrinolytics like tranexamic acid 500mg thrice a day for 3-5 days when bleeding occurs and its safe to use completely and at the same time to try adopting a stress free life style by taking to relaxation exercises or meditation / yoga activities which can calm you down and help remain stress free.
If the bleeding pattern is not changes for another 2-3 months and you still experience the same heavy bleeding pattern, then you may get implanon removed as it may not be the right choice of contraception for you then.
I hoep i have answered your query in detail,
Wishing you good health,
Regards
