Urethral caruncles are benign, distal urethral lesions that are most commonly found in postmenopausal women. Most urethral caruncles can be treated conservatively with warm sitz baths and vaginal estrogen replacement. Surgical intervention may be indicated for patients with larger symptomatic lesions and for those with uncertain diagnoses. Induration, failure to respond to conservative therapy, atypical appearance, or growth over time may be indications for excisional biopsy. However in your case your symptoms are primarily itching and yeast has been ruled out. So likely cause remains dryness and often use of tight undergarments which can cause frictional injury locally and often accompanied by itching during the healing process.
Please avoid using any soap locally but rather you may just use plain water and sitz baths to help. Also you may use oestrogen cream like Estradiol 1% or Evalon cream for local application over the caruncle and locally twice a day, this should help you re-epithelializeand hence reduce itching. You may discuss the option with your primary health consultant and take an informed decision. Also important is to wear loose, cotton undergarments to avoid moisture induced or friction induced lesions.
