Brief Answer:
Need further evaluation
Detailed Answer:
Hello dear,welcome to HCM forum.
I understand your concern.
Pain, along with swelling without a history of local trauma usually indicates a soft tissue infection. The progressive nature of pain is also suggestive of an ongoing infection.
In the area behind ear, this may indicate simple causes like boils,or more important causes like mastoiditis,an infection of the mastoid cavity that needs urgent initiation of antibiotic treatment.
A local examination will help in assessment.
Kindly get back to your family doctor or an ENT surgeon.
Meanwhile,you can continue the analgesics.
Hope this will answer your query.
Wishing for a faster cure.
Thank you.

Brief Answer:
Mastoiditis to be ruled out
Detailed Answer:
Hi dear,
If the swelling is more diffuse, and you are sure of not having superficial infections like boils,as I already told,we have to rule out deep infections like mastoiditis.
Your doctor will guide in this.
Depending on the examination findings, you may be asked for investigations like x-ray or CT scanning.
Hope you got cleared now.
Thank you.
