Brief Answer:
Torsion testis or Acute Infection as the most probable cause.

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Male/55 - serious pain - started in testicle - lower abdomen - vomiting earlier - tried Ibuprofen ....

Such a severe pain in one testicle with referred pain to abdomen is indicative of torsion of the testicle as the most probable cause.
Another cause can be severe epididymo-orchitis, that is infection and inflammation of the testicle and the epididymis.
The vomiting is usually a reflex one as a response to any severe pain.
I would advise you the following in such a situation:
Urgent Ultrasound and color doppler of the scrotum and the abdomen to know the exact cause of the pains thus getting a chance to get an urgent relief by surgery if torsion is diagnosed and medical management if infection is the cause.
Also needed is the tests of blood: CBC, sugar kidney function as as needed.
Urine: Sample should be sent for routine, microscopy, culture and sensitivity before the first dose of an antibiotic.

This way an earliest possible diagnosis is needed, please go to ER asap.
I hope this answers your query, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
