Question: My friend has swollen toes and swelling on the top of his foot. He is in pain and cannot put much pressure on his foot. His foot is starting to peel and it is even painful to put on a sock. What do you think?

Going by above description it appears that your friend is suffering from effects of decrease in circulation either due to direct or indirect disease like diabetes ,peripheral arterial disease or cellulitis in the foot.
I would suggest Limb Elevation , antibiotics , X ray of the affected part and consultation with a foot surgeon or your regular physician.
