Your face looks very red and with the pains you have reported, I would want to think there is some sort of superficial infection associated to the underlying problem that needs to be addressed. This rash could be acne, rosacea or some other thing. My immediate thoughts would have been super infected acne before rosacea seen the distribution. Rosacea is mostly concentrated on the nose in most cases. However, I will also like to ask that this be considered for lupus related rash.
This is a difficult call to make. it would be best to follow up with a dermatologist who would make a plan and follow up with you. When a disease is not typical, a trial of treatment ay be necessary. Should you be well treated for acne with no improvement, switching to other possible causes or following up with more investigations would be helpful.

I will suggest you take the picture along. Also, you may leave the house with the make up one but make sure you cleanse everything in the restroom before showing up for your appointment. it will not he helpful to go to a dermatologist and not let him/her see the exact nature of your problem.
I understand what a blow this must have been to your self confidence. Feel comfortable with it and know you matter most to yourself and not others and if you can see the real you, that's what matters.

From the information you have provided, I am afraid you may be having episodes of orthostatic hypotension. This is to ay the blood pressure drops very low especially when you switch from a lying position to an upright position. The most common cause of this is dehydration.
However, orthostatic hypotension would not persist al the way to the ER where your blood pressure can be measured and detected. What I will suggest is that you need to drink more fluids than you currently do. Low blood pressure is first a problem of fluids balance.. There are other causes like adrenal gland failure but this will not produce episodic hypotension but rather a continued state of low blood pressure.
For now, drink much water to compensate for losses. Transition from the sitting to an upright positing gradually.
