What Causes Recurrent Holes In The Nasal Septum?

Question: Hello! I am overweight and am currently concerned about a hole that has developed in a fat roll. This is the second time the hole has appeared, and both times something popped. The pop didn't worry me because it was a small tear or puncture-sized where the fluid eacaped, but then the skin that had been covering whatever popped just disappeared into this open hole that stayed for at least a week. The hole never made a scab as it's under a roll and little air can get there. Again, this is the second time in the same spot, and it was very difficult to get it to heal last time (and apparently r didn't even heal very well since it's back) I'm concerned, please advise?

Recurrence of the hole at the same spot in a fat role after difficult healing last time indicates the following:
The fat necrosed in the last incidence and that has not come out, works as a foreign body once devitalized. The skin might have covered it last time and the part acted as a catalyst to have recurrence of the abscess/cellulitis and the pressure caused the popping and ulcer formation. This acts as a sinus now.
Another reason can be a retained cyst that infected last time.
Whatever the cause, this usually needs an active treatment.
Exploration and removal of any devitalized tissue is the mainstay of proper management and avoidance of recurrence.
Get a consultation of a General Surgeon for clinical examination, diagnosis and proper management. Please discuss these points with him to get a proper and an early treatment.
