Redness on scrotum is due to many reasons.
Most common is scrotal dermatitis, Tinea cruris, Lichen simplex chronicus.
Others may be erythrasma, psoriasis and many more differentials.

It looks like Lichen planus. It is an inflammatory skin condition, characterized by a non-infectious flat topped rash. These are purple, polygonal, pruritic, papules.
Is your lesion is itchy?
Any other part of body affected? Tongue, nail etc
Duration of disease?
The other close differential is Inverse psoriasis.
These two lesion need differentiation by dermatopathology.

It is not itchy, no other parts of body, I discovered it 5 days ago, i believe it started a day or two after recieving rough oral sex when woman sucked that testicle rather rough.

As it is just 6-7 days after sex so very less chances of STD. No pus is seen on photograph - less chances of bacterial infection.
I am in favour of Lichen planus and will advise to go for dermatopathology for confirmation of disease.

Topical steroid with high potency and emollient is the first line of therapy.
In recalcitrant cases tapering dose of oral steroid and calcineurin inhibitor is also given.
These all are prescription drug, consult your local dermatologist to get this.
