After going through your details, only reason behind your symptoms I could think is MVP (Mitral valve prolapse) with regurgitation. You need to go for reevaluation of your cardiac status. You might have moved from mild to little higher category. Meantime please avoid severe exertion.
Visit your cardiac specialist. Get echocardiogram and other investigations done to assess your heart status.
