Thanks for your question.
I can understand your concern.
After going through your question, I would like to advise you to monitor blood sugar levels at home by using glucometer (SMBG- Self Monitoring of Blood Glucose) as follow-
- Fasting (Before breakfast)
- 2 Hours after breakfast.
- Before lunch.
- 2 hours after lunch.
- Before Dinner.
- 2 hours after dinner.
- Early morning 3am.
It is usually advisable to do as much as readings possible from all of above readings daily.
Otherwise at least 2 or 3 reading should be done daily.
If required additional readings should be done in case there are symptoms of low blood sugar (hypoglycemia), in that case you should also notice date and time of incidence any change in meal pattern, increased physical activity on that particular day.
Depending upon weekly chart your doctor can adjust dose of your current insulin Humalog mix & Glimepiride or may change your insulin to basal bolus regimen (using short acting and long acting insulin) according to Blood sugar level readings.
In your case swelling over body may indicate
1) Worsening of congestive heart disease.
2) Kidney problem.
3) Inadequate thyroid control.
So it is better to consult your treating doctor, discuss all these issues with him, after detailed clinical examination there may be need for further evaluation like
1) Blood tests: Complete blood count, kidney and liver function tests, serum electrolytes, pro BNP, Thyroid function test.
2) Urine examination for albumin.
3) EKG & 2DEcho of heart.
4) Ultrasound of abdomen and pelvis for kidneys.
6) Chest xray.
