Question: My sister is 30 years old. About a month ago she had this problem. She feels urge to go to toilet with chills but nothing passes. This was there for about two weeks and she became better for about a week. Then she developed a diarrhea. yesterday she went to toilet 15 times. She has no other symptom like fever etc. All her blood reports CRP and stool tests are normal. I am wondering what could this be?When the first time the symptom developed she had visited a farm with cattle and dogs.

Hi,

I have gone through the medical history of your sister and understood your concern.

Let me recapitalize her case:
- feeling urge to go to the toilet without having a bowel movement which was associated with chills
- going on for two weeks
- getting better for a week
- after that developing diarrhea (yesterday 15 times)

I was reviewing all her blood tests and stool reports and found nothing. Everything came clear, which excludes infectious disease of the GI tract.

Then, I'd like to know if she is having problems with constipation. Sometimes, the lack of bowel movement (bm) can give the presentation of her symptoms.

To my opinion, we should investigate her bowels more specifically. For this, I recommend to :
- get a physical/digital examination of her anal area to exclude : hemorrhoids? anal prolapse?
- colonoscopy to evaluate the changes in her mucosa
- culture of stool to exclude candida infection

Get her further checked by GI specialist and ask for the tests as advised earlier.

Meanwhile, to ease her diarrhea:
- take probiotics to stabilize intestinal flora
- increase fluids intake with OTC ORS
- take tasectan
