Constipation can be a common complaint in the elderly patient, especially if they are on multiple drugs like Calcium channel blockers that have constipation as a side effect. It can occur in association with long term diabetes as well. The irregular bowels is a sign of concern. A stool R/E if possible should be obtained to look for any occult blood. A physical exam to look for signs of obstruction and peritonitis would be required. A colonoscopy could be done if the findings are confusing and the symptoms persist.

Thanks for the clarification.
Renal function tests including Urea and creatinine need to be done to rule out any acute insult to the kidneys. The oliguria ( less than normal urine) needs to be investigated further to find the exact cause. A urine R/E (routine exam) and an ultrasound abdomen and pelvis would also be required to have a better idea. She would be required to keep track of her 24 hour urine output for a few days. If that would not be possible, depending on her illness she may need to be catheterized.
