What Causes Tingling Sensation And Numbness In The Left Hand?

Question: About four weeks ago I started developing numbness in my left hand and slightly up my left arm. On a scale of 1 to 10 it is about a 5-7. It feels like my hand is asleep. It tingles, especially in the forefinger and thumb. I also have a slight sensation in my right hand, but much less ...maybe a 1-3,

1)Do you have neck pain, radiating to arms?
2)Do you have Spinal/disc disease?
3)Do you have Diabetes or any other known illness?
4)Do you have any addictions?

This can be nerves related issue or spinal issue causing the symptoms of radiculopathy/pinched nerves.I shall be able to guide well further with above details.
