Question: Hi Doctor
My sister needs a second opinion on her attached reports. What do you suggest/ recommend ?

Doctor's Prescription
Vitamin D
Vitamin B12
Callcium
Alkaline Phosphate
Haemoglobin
Lower back X-Ray
Lower Back MRI
Last tests on Vitamin D & Vitamin B12 were conducted 18.03.2016. Report attached.

Her Doctor suspects Osteomalacia which is Calcium and Vitamin D deficiency in adult. But her values appear good.

Issues and action so far:

1. Pain in lower back. Started since 2nd pregnancy trimester, from Apr'15.
2. Lying on back, straight, was very difficult for longer period, never slept on back for more than an year post Apr'15 due to pain. Pain increased while lying or getting up from bed. It's still there but, quite less now.
2. Met ortho in Dec'15 as pain persisted even after delivery. He advised to wait for an year as ruptured membranes during pregnancy and delivery takes almost that time to heal. Advised no forward bending, weight lifting as pain was increasing with it. Have taken pain killers whenever pain was unbearable. Spine has curved a bit more after delivery.
3. Started some mild exercises for few months. Did physiotherapy after ortho and gynae advise in Sept'16 end for a week. After therapy and still continued exercises, muscle spasm improved and am currently able to sleep on back with legs folded as advised. Flat footwear stopped, weight lifting (even holding baby for longer period as he weighs almost 12 kg) was refused till back strengthens.
4. Forward bending even for 30 seconds increases pain. Exercises are helping but, I need a permanent cure from pain.
5. Some recent issues are sounds from bones on body movement. Upper back also has a little pain now and then.

Doctor has advised that my Vitamin D is within range but, decreasing and hence, pain. Was 55 in Apr'16, now, 33 in Dec'16. Lifestyle needs to be improved, exercises needs to be continued non-stop, step outside house for sunlight, walk should be a routine part.

Need opinion on this information and tests conducted. Thanks

I have read the problems that she has been facing since before the delivery till now.

I understand this is a very difficult time to go through and she is suffering much.

I would like to see her xrays of lumbosacral spine AP view and Flexion and Extension lateral views and if possible MRI as well.

She could be having slip disc or Spine instability. Each of these conditions have different management so it is upstand to get the Xray and MRI to confirm.
