What Does CA Of 125 Indicate?

Question: Dear Doctor, I had contacted you earlier but by the time I got back with the reports of CA 125 as suggested by you for my wife, the conversation was closed.

Two things :
1) She has a cyst in her left ovary, size 31 x 28 mm. Reduced from the previous size of 38 x 32 mm.
2) CA 125 report is 218.

Pls advise.

The CA-125 report is positive.

But what I recollect you had written

" i have a cyst in my left ovary. During my check up in May it had shown signs of subsiding (LO size 43 x 31 mm). However, after my ultrasound in XXXXXXX it showed an increase (LO size 61 x 39 mm; 2 cysts reported 23 x 20 mm and 35 x 28 mm)."

So when is the cyst 31 x 28mm now or earlier. Also when was she 38 x 32 mm ?

Kindly give me the EXACT sizes of the cyst and number of cysts and also THE EXACT CORRESPONDING DATES to them.

Thanks for recollecting.

Yes, pls ignore the earlier reports. I am told by somebody who is one of the better doctors for an ultrasound in XXXXXXX that they were wrongly interpreted earlier.

He has reported a single cyst, size of 31 x 28 mm.
The CA 125 report is 218.

This doctor here has said that cancerous levels are above 600, though my wife's cyst could be malignant. He has suggested she be put on antibiotics as suggested by a gyane and then a follow-up for the CA 125 test in couple of weeks. If results drop then fine, else he was suggesting a laprscopic surgery.

Pls advise. Quite frankly, I am not worried about my wife not being able to conceive in future but her wellness is my primary & only concern.

Yes, what your doctor has advised is absolutely right.

Though her CA-125 levels are high normally we suspect cancer above 300 (not 600)

. Anyways its below that.

But CA-125 is not a 100% accurate test. So we can only suspect with this test but not confirm.

Yes,

1) Start her on antibiotics. Then repeat sonography in 3-4 weeks.

If size does not reduce and CA-125 also stays same or increases then she needs a laparoscopic surgery and frozen biopsy.

After she is treated she can conceive.

Pls advise on the antibiotics she should start a course on.

Just to give you a background of medication she has done :
a) Primolute N - 1 tablet, thrice a day for 3 months. Period : Mar to Apr.
b) Meprate - 1 tablet a day for 20 days. Period : XXXXXXX

Antibiotics to be started :

Doxycycline 100 mg two times a day before breakfast and before dinner. This is for 14 days

Metronidazole 400 mg three times a day before food for 5 days.

Pantoprazole 40 mg once day before breakfast for 14 days.

Then get a sonography after 4 weeks from starting the antibiotics and let me know the details.
