It can be elevated in infections, inflammation like TB, rheumatoid arthritis, osteoarthritis, giant cell arteritis etc. Therefore it is not possible to specify pneumonia / TB / cancer based on elevated ESR alone. It has to be correlated with clinical findings.
Have you had any infection or do you have any chronic disease?
A little clinical history can help to figure out the cause of elevated ESR.
