I have read through your query in detail.
With my experience in doing testis ultrasounds regularly, it is least likely for a worm to enter the testis from the urethra as mentioned by you. I am not ruling out the possibility but trying to get the facts that a worm in the urethra will go the the urinary bladder and it is very difficult for it to enter the small tube connecting the testis.
Besides the ultrasound scan shows that no such worm is present. Commonly, there can be conditions in the testis concerning blood flow through the venous channels which give a feeling of worms and this is known as varicose veins. If present, this would also have been found on ultrasound.
In rare occurrences, it can also be that you have a mild infection in the testis and it is causing pain and discomfort.
It is not possible for a worm described by you to be present in the urethra for a year and live off the sperm. This is because the urine washes the urethra many times daily and worm would have also been washed out. Also as mentioned above, the urinary bladder is a much easy access to the worm than the testis.

If you have been sexually active in the recent times and then your problems have started then it is always good to get checked for STDs.
Since you have not mentioned in detail about the worm, it is difficult to say if the worm can stay in the bladder for a year. However, any condition arising in the bladder due to the worm will be picked up on ultrasound scan. There are worms like urinary bladder fluke which survives in the bladder.
