What Does The Below Reports Indicate?

Question: I found TSH test results abnormal and went for preventive health check-up. I have attached both detailed reports and wants this to be reviewed by physician and suggest immediate next steps on the basis these lab results if any required. I have listed down some specific questions on the basis of these attached lab reports.

1. Thyroid profile test has TSH above normal range in both reports done in Feb(7.43) and Apr(6.19) 2015. Is this something need immediate attention like start medication or this is manageable? just to add here I am overweight (BMI 33.67).
2. Vitamin D level in report sounds alarming? what should I do?
3. Vitamin B12 is less than normal level. what should I do to make it normal?
4. There are multiple parameters outside of normal range in CBC profile in attached lab test results. Is there anything need immediate action, any suggestion to bring these in normal range?
5. LFT test results has (ALT / SGPT) slightly above normal range value? is there any suggestion for that?
6. KFT has Urea slightly below normal range value. is this fine?

Hello,
I had gone through your reports and i can understand your concern very well.
Coming back to your queries, i must say that you had undergone almost all important blood examinations.

First of all, you need to reduce your weight by at least 20 kg. I know its quite difficult, but you will have to, because half of your problems will go away by just reducing your weight.

1. Your TSH is on the higher side on both the occasions, so you are having hypothyroidism and you need to start treatment in the form of levothyroxine as soon as possible to maintain the normal levels. As its a prescription medicine, you need to consult a physician to confirm the diagnosis and start the treatment. I would also suggest you to do free T3 and free T4 levels rather than total T3 and total T4. After starting the treatment, you can check the levels of TSH every 6 to 9 months to adjust the dose of thyroxine.

2. Your vitamin D level is very low and it can cause osteomalacia (softening of bones), which makes bones more prone to fracture even with trivial trauma. Vitamin D is one of the fat soluble vitamin and concentration of less than 12 ng/ml needs treatment, so in your case level is way below and you definitely needs treatment. Treatment usually consists of vitamin D rich diet (milk and dairy products, fish liver oil, etc.) , and supplements (in the form of vitamin D3). Also you will have to spend much time in the sun before 10 am in the morning. In your case, the most common cause of lower vitamin D is obesity, because vitamin D is taken up from blood by fat cells and alters its release into circulation.

3. Your vitamin B12 is just below than normal and does not require any kind of treatment, but you need to take B12 rich food like milk and milk products, eggs, non-veg foods, etc. Measure the level after 6 months and you will definitely find it normal.

4. In CBC profile, mostly everything is normal and you need to take iron rich food (green leafy vegetables, jaggery, etc.) more to raise your Hb. All other parameters are more or less normal and no need to take any action for that. RDW is high due to lower B12/folic acid and it will be corrected automatically after restoring B12 levels, no need to worry about that.

5. In LFT, ALT is slightly high, but not that much and it might be due to changes of fatty liver because of obesity, so again needs to loose weight. Its good that your lipid profile and sugar are within normal range.

6. Low urea has usually no significance, so you need not to be bothered much about it.

So at the end, i strongly recommend you to cut down your weight by life style modifications like daily walking, join a gym, eat healthy diet, avoid oily and junk food, do meditations, take a 6 to 8 hour sound sleep, etc.
