default
Posted on Thu, 7 Jul 2016
Question: Hello Doctor,

I have done High Resolution Aoscopy (HRA) with Biopsies for anal canal and anal margin.
The biopsy reports came back and I could not meet the surgeon to explain the reports.
So, my question is what are the findings of the report?

Brief Answer:
No cancer cells present.

Detailed Answer:
Hello!

Thank you for the query.

According to the report only inflammatory changes were found. No cancerous cells, no HPV infection (Human Pappiloma Virus). Just an inflammation most likely due to some superficial injury (hard stool can cause such injury for example).

So this is a very good result and you should not be worried.

Hope this helps. Feel free to ask further questions.
Regards.

Thank you doctor for your prompt response.

Note: I am having internal and external hemorrhoids for about a year.

1. Can chronic inflammatory changes lead to malignancy? Does it require treatment?

2. Can perinuclear halo be sign for HPV? even though P16, P53 and HPV test are negative? in case if there is possibility that these test missed HPV.

3. Does Hyperkeratosis need treatment? does it progress to more serious condition?

4. how long does it take for the biopsy wound in the anal canal to be totally healed?

Regards,
doctor

Brief Answer:
Please find the answers below.

Detailed Answer:
1. Yes, chronic inflammatory changes are known factor of a cancer. Especially in case of inflammatory bowels disease (like Crohns disease or Ulcerative Colitis). But it is also known that the inflammatory process must be present for about 7 years to increase the risk of colorectal cancer. Your inflammation does not require treatment. Its a part of healing.

2. Yes it can be a sign of HPV. HPV test detects virus DNA. It is very sensitive test (detects the virus in more than 90% infected cases). So it is hardly possible that the test is false negative.

3. No, hyperkeratosis itself does not need treatment. Dysplasia is something precancerous and something that should be treated.

4. Anal mucosa usually heals in couple of days.

Regards.

Brief Answer:
This does not look serious.

Detailed Answer:
No, it does not look serious. Just an inflammation. Can be due to a virus. If you do not smoke, generally you do not have to be worried about any cancer in this location.
I suggest you to try probiotic intake, increase natural yogurt intake and rinse your mouth with Octeniline. This should greatly help with this lesions.
