What Does This Thyroid Test Report Indicate?

Question: Hi,
58 y.o female, I have TSH 0.67 and TPO >1000. I am not taking any meds, and asymptomatic. Is this Hashimoto? what labs or imaging studies to do next?

Yes, it is Hashimoto thyroiditis!

I would recommend to check also FT4 snd FT3 levels and do an ultrasound of the thyroid gland as patients with Hashimoto thyroiditis has a greater predisposition to do thyroid nodules.

If all of them results to be normal, than you need to check TSH level every 6 months.
Once it is over 4.2 mUi/ ml and you have symptoms of hypothyroidism, levothyroxine should be started.

Regarding to hormone replacement therapy it is recommended to be started at the first years of menopause, maximum within the first 5 years.

The best therapy will be decided after a detailed clinical examination and evaluation of your personal and familiar history for cardiac, malignant and other pathologies.
