What Does This Ultrasound Of Uterus Indicate?

Question: I have an FSH of 9.9 and estradiol of 330. I am 44 years old. Erratic periods for 6 months after positive pregnancy test. Negative Quant HCG. I should now be on my period, but have only the sensation of being on period with cramps and bloating without any bleeding. Any ideas?
I also should note: normal transvaginal pelvic US previous to all of this from abdominal bloating and increasing painful periods. (but my periods were like clockwork at the time... (The US could not see my L ovary... which I think is probably good... not like a huge mass if that can not find it : )


Uterus: Measures 7.4 x 5.1 x 4.3 cm. No discrete uterine mass
is visualized. The endometrial echo complex measures 10 mm in
thickness.

Right Ovary: Measures 2.4 x 2.0 x 3.0 cm. Appears within normal
limits without focal mass. The ovary demonstrates normal internal
color Doppler flow without sonographic evidence for torsion.

Left Ovary: Not identified on this examination.

Miscellaneous: There is trace amount of free fluid adjacent to
the uterus and left adnexa.

Detailed Answer:
When I see someone like this in my practice I order the following tests

CBC
CMP
TSH
Free T4
AMH
Inhibin B
FSH
LH
Estradiol

I see you have had your FSH and Estradiol done once, but I generally repeat it a month apart.

I have noted your negative b hCG and ultrasound findings.

So once pregnancy is 100% ruled out, one needs to make sure the thyroid is fine (TSH and free T4 blood tests), and there are no other medical conditions evident on routine blood counts, liver and kidney function tests, electrolytes and glucose tests (part of CBC and CMP).
The FSH, LH, Inhibin B, AMH and estradiol will give further idea about the hormone function status. I would even consider checking blood levels of Prolactin.

None of these tests require fasting and can be done at anytime of the day.

Further, it is always advisable to see a gynecologist for an in-person examination and interpretation of lab results in the specific context of your complaints.
