TSH is produced by pituitary gland and sometimes ( especially when recovering from any illness) you can get a high value which can become normal after sometime.
Your wife's current TSH is 3.39 and the safest TSH value recommended before pregnancy is 2.5.

thanks i have t4 but dont have anti TPO does it has to be fasting pls see her attached reports

She also has an elevated insulin level 2 hrs after glucose load during the gucose tolerene test. This will improve with exercise and weight loss. This also will help PCOD and infertility

Her Sugar levels are not bad. However, Metformin ( glucophage) may improve the insulin levels and also helps with fertiliy rates. I recommend a small dose of Thyroxine 25 mcg for her and repeat the TSH and Free T4 after a month. Along with that, you can also do an Anti TPO
