Posted on Sat, 11 Oct 2014
Question: I am going on Pilgrimage On Monday. I was due to have my period on the 12th September. Wanted to bring period forward. Took primolut from day 15 of Cycle. ( 5 mg three times a day for five days. Then one tablet a day for two days thereafter to plan my cycle). It is now 6 days after I've taken last tablet with no period. Am very concerned that my cycle is messed Up. And that i will have a period on pilgrimage days which takes place about two weeks from now.
what could the reasons for no bleeding be

Brief Answer:
You started medicines too early

Detailed Answer:
Hello XXXX
Thanks for contacting and trusting Healthcare magic.

Tou have taken primolut n from day 15 itself which was really early to take as secretory phase of endometrial cycle had just started . you could have actually allowed your period to take its normal course as your plan from pilgrimage is two weeks from now and you would have been period free by that time already.

Ideally primolut n or medroxyprogesterone acetate are used for delaying the periods as long as required , with religious reasons topping the list. but in such cases always, it has been seen that the resumption of menses is always delayed and the menstruation ranges from spotting to scanty menses initially with withdrawal bleed and then may resume again with breakthrough bleeding after another 15 days. so this can continue for next two cycles.

In your case now, if you plan to go after 2 weeks then , you can just wait for your menses to resume now . Don't restart primolut-n. it should start by another 7 days , ideally they occur within next 15 days of last dose.

I hope i have answered your query.

Thank you. Pilgrimage starts from the 3rd to the 7th of October . Is it likely that I will have a period then? If so should I re take primulot and when should I start. I have Very very slight spotting which starts then disappears totally.

Brief Answer:
your spotting is infact your period this time

Detailed Answer:
Hello XXXX
thanks for follow up.

you didnt mention about spotting in your question earlier. i was expecting the same , why because you took the primolut at the start of secretory phase when endometrium was to mature , and then you opted for shedding it off with primolut, so the amount of bleeding expected was less to scanty to spotting. so as you have spotted already , please understand that this is your period for this cycle and the start day of spotting becomes the first day of the period in this cycle.

so you are in the start of new period already. as far as not bleeding during pilgrimage is concerned., as i advise my patients , i can suggest instead of prinolut n, you can take TAB MEDROXYPROGESTERONE ACETATE (MPA)10mg twice a day daily from day 21 now of this cycle till the date you dont want your period, say 10th october. please dont miss any pill at all, as you may bleed then.
Once your pilgrimage is over you may stop it and then you can have your menses resume within 7-10 days , and may be heavy. excess blood loss can be controlled easily by antifibrinolytics.

the MPA can be prescribed by your gynae and considered for use at your end.

p.s:- your spotting is your period this time.

i hope i have answered your query in detail.
regards,
DR.Sameer Kumar
