
Red Spots On Leg, Blood In Semen, Unprotected Sex. STD Or Cancer?

A drop of blood in sperm during masturbation occurred twice a few days after intimacy. Age 23, Caucasian male. Grandfather died of prostate cancer. Girlfriend never been tested for STDs, had unprotected sex with her. She has had a history of unprotected sex with two other males. What could it be? Sort of STD? Cancer? Something else?

Since you have sensitive skin, so these red flat spots can be due to simple dermatitis skin, friction rub causing chafing or fungal infection which can be transmitted by physical contact with an effected person but it is itchy and moist in nature. Wear cotton clothes and use a mild soap like Dove. If these red spots start getting itchy then fungal infections like ringworm have to be ruled out.
However what concerns me is the presence of blood on a few occasions in semen. It is called hematospermia and the primary cause is sexually transmitted diseases like herpes, Chlamydia, cytomegalovirus and trichominiasis. Since you had unprotected intercourse with a girl who has not got her STD status checked, it becomes utmost important for you to get these STD’s ruled out. You need to get a culture of the urethral swab and discharge and blood tests for herpes done to rule out these STD’s.
If the STD panel comes out all clear then other possibilities have to be investigated. It can occur due to many conditions such as swellings (inflammations), cysts, stones & other obstructive lesions, and benign or non-benign tumors of the organs of the genitourinary system (involving the bladder, urethra, the testicles, the seminal vesicles, the epididymis, the prostate gland etc).
Investigations which you then need to get done include blood tests, routine & culture tests of urine & semen, ultrasound & MRI scans of genitourinary system, and histopathological tests (FNAC, biopsy etc).
