Are Both HCD And MSIR Opiods? What Can Be Done If There Is Difficulty To Switch To MSIR From HCD?

I am a hospice nurse with a patient who is on Duragesic 100 mcg and HCD 10/325 tab i to 2 q 4hr prn pain, he is having difficulty swallowing and our md is wanting to switch off the HCD to MSIR 15 mgm q 3h, they are both opiods, correct? wife is having difficulty with the switch, how can I help convince

hi,
There have been reports of difficulty of swallowing with Duragesic (fentalyl- a synthetic opioids analogue). The current problem may be a part of it. I would advice switching it, for a therapeutic trial. But your MD might have different reasons for swithching as such.
Regarding, difficulty for her wife having difficulty for the switch, focus on telling some medication in his list develop difficulties of swallowing in some people, and hence its been swithched.
