Hi,

Painless haematuria (blood in urine) can be due to various causes like urinary tract infections (UTI), kidney infections, prostate disease, bladder disease, chronic kidney disease, and tumors. It may also be due to heavy exercise, in which case, it is shortlasting. Heavy smoking by itself cannot cause haematuria, but over years, it does increase the risk of other diseases.

I feel your boyfriend should not neglect this condition and should go to see a doctor as soon as possible. In addition, he will do well to consider quitting smoking, more so as he already has a history of COPD.

Hope to have been of some help.
