Can Constant Swelling Around The Incision Site Be Due To Incisional Hernia?

I had an appendectomy 3 weeks ago. I went back to the doctor about 10 days after the operation who confirmed that the incision was healing well and that there was no infection. However, I have this swelliong around the area of the incision which started shortly after the operation. It is bulging out and it gets quite hard sometimes and uncomfortable and does not appear to be going down. Not sure whether this is normal or whether I should be concerned. Could it be an incisional hernia?
