Hi,

Anginal pain is characteristically central or retrosternal/substernal; pain may radiate to either arm (LT> RT), hand or wrist, shoulders, less often to the back of the chest. Any of these places of reference may be involved without discomfort in the chest. The relationship to exertion is then the indication that the pain is probably of cardiac origin.
History of lifting unusual weight (cot) and local tenderness overhand (on pressing the hand feels little pain) suggest musculoskeletal pain and pain is of non-cardiac origin.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
