Chest X-ray, Calcific Density, Left Apex. Calcified Granuloma?
Can you explain the findings of my friend s Chest X-ray if he is fit to work? There is ovoid calcific density seen in the left apex. Heart is not enlargde. Diaphragm and bony thorax are unremarkable. Impression: Calcific density, left apex. Calcified Granuloma?
Calcific density in the apex of the lung means that there was some infection, mostly chronic which has now healed and has got calcified. Generally calcification also indicates benign nature of the illness. If your friend is not having any symptoms there is no need for any more tests.
Calified granuloma in left apex usually a sequelae of tuberculosis in India. So, ask him that he had any past history of tuberculosis? If not then it must be investigated as there may be presence of dormant tb bacilli in calcified granuloma. If he is asymptomatic then there is no worry. But if he has fever, cough etc then go for a sputum for AFB smear, Mantoux test with 5 TU and a CT SCAN Thorax with contrast.
As long as there is no bloody cough, pain in left apex/ or SOB / or restricted movement and pain on physical work , your friend can work.
However you need to do a culture of sputum for bacteial/ fungal growth in a granulomatous lesion in a presentation like your friends x-ray.
