Diabetes, High BP And Triglycerides, Gastric, No Thyroid Issues, Tiredness, Lack Of Sleep. Explain Reason

I am diabetic/ high BP for last 2.5 years. age 48 yrs. Controlled triglyceride, BP with medicine. Excessive Gas problem. For last 3 months fasting= 130, PP= 160. No thyroid for any other problem.
1- whether can have heart problem?
2- How to check and what tests required
3- feel very low, lethargic, less energy, less sleep

first of all try to bring the fasting blood sugar within normal range by increasing the dose of insulin and controlling your diet with regular exercise . get your glycosylated haemoglobin estimation done , routine examination urine to rune out haematuria and proteinuria , to rule out cardiac and renal cause of lethargy you can go for electrocardiography and echocardiography, renal function test including complete haemogram , for monitoring cardiac pathology in future you can keep your eyes on serum LDL , HDL ,total cholesterol, homocysteine and hsCRP level.
