Diabetic, Difficult To Manage Pp2bs After Meals. On Glimy 2. Any Suggestions?

Hello doctor, I am a diabetin from past 1 year. I am 35 y old male. My mother is also a diabetic. I am taking glimy 2 everyday before breakfast. My recent HbA1C is 7.2. After lunch it is difficult to manage pp2bs. It is in the range of 160-200. My doctod advised that I should take glimy 2 twice one is before breakfast and another is before dinner. I am doing a morning walk everyday about 3.5 km. I am controlling my diet also. But sometime due to less age diet is difficult to control. I am not over weight. My height is 5'8" and weight 72. Can you make some suggestion for me.

Thanks for posting your query.
Diabetes is due to progressive loss or destruction of beta cells in endocrinal part of pancreas.
Diabetes is a multi organ disease with involvement of almost all organs of body due to its complications.
Blood sugar both fasting and postprandial must be in controlled range.
You should consult with with internal medicine specialist/ endocrinologist and should go for thorough check up.
You should continue Glimy 2 BD as per advised of your treating doctor.
In my personal opinion, you can also take vildagliptin 50 mg OD along with voglibose 0.2 mg along with meals.
You should also follow dietary advice as suggested by your treating doctor or dietitian.
You should also perform increased physical activity.

HbA1C 7.2 is moderate control of Diabetes. In type 2 diabetes, insulin resistance will usually be present. It means that the action of insulin is reduced. Since you are already taking Glimipiride to increase insulin secretion, an addition of Metformin 500 mg along with it would be more effective. Metformin will increase insulin action.

Also the advantage of Metformin is that it will reduce the intestinal absorption of carbohydrate and will reduce your appetite. Since it is difficult for you to control the diet, Metformin will help you a great deal. Also it will not result in hypoglycemia like Glimipiride. You should consult your treating Dr about this drug.

But this drug can result in nausea, metallic taste and bloating sensation as side effects. You can take small frequent meals as that will reduce the sudden spike in post prandial sugar. Include lot of vegetables in your diet as that will be more filling.

Hello,
My sincerest advise to you would be to take appropriate diet measures along with exercises and medications.
If you are not able to control your diet and calorie intake,your blood sugar levels would not be controlled properly.More over you are a very young person and as your age advances,these oral anti diabetic drugs would keep on loosing their efficacy and one day you would have to switch over to injection insulin.This happens because of progressive loss of beta cells of pancreas which are responsible for production of insulin in response to the intake of oral anti diabetic drugs.
So the best would be to keep a control on your diet by taking 6 small meals a day rather than 3 big meals.
Avoid sugar,sweets,colas,junk foods,deep fried foods etc.
The more control you have on your diet,the less dose of medicines would be needed.
Make sure that your blood sugar,blood pressure and lipid levels are controlled meticulously so as to avoid various types of future micro as well as Macrovascular complications.
As far as exercises are concerned,since you are young,you should opt for brisk walk for atleast 45 minutes a day.
Take medicines regularly as advised by your doctor.

Hi,
thanks for the query to H.C.M. Forum.
Hb A 1 C is 7.2 %.
Your mother is a known case of diabetes.
You are a known case of diabetes , but your weight is only 72 kg and this is good weight but concern is that this is not a good thing , how not down.
PP level is 160-200 daily.
So I would like to advice you that first of all please get in U/S abdomen with emphasis over "PANCREAS" to rule out any disease in this gland( This is very important for you).
Now don't take it easy as diabetes is most dangerous disease , so be careful , take few steps as,
Control diet at any cost irrespective of age.
Regular exercises which burns more calories than you need.
Sheer walk of 4 km is not sufficient for you until it is very very fast.
One more thing to note ,if possible , don't take tension of any thing.
