Hi Doc, i just visited the doctor today because i had a positive pregnancy test. The result of my ultra sound is a. Anteverted, non-gravid uterus , no retained products b. pelvic free fluid. Ruptured ovarian cyst or ectopic pregnancy Can an ectopic pregnancy be spotted two weeks after your last sexual intercourse? Please help me

An intrauterine pregnancy can be diagnosed by a trans-vaginal scan at about 32 days gestational age when the hCG levels are at 1000 I.U./L.-2000 I.U./L. The absence of evidence of intrauterine pregnancy, along with other features like visible fluid in the pouch of Douglas go in favor of an extrauterine pregnancy, when the hCG levels are more than 1000 I.U./L. I advice you to correlate your findings with your serum hCG levels and follow up with another scan as sometimes intrauterine gestational sac appears later on scan. Pelvic free fluid could be due to a ruptured ovarian cyst also, but this finding needs to be clinically correlated. Please see your gynaecologist for further management. Take care.
in case of ruptured ectopic pregnancy you can see empty uterus with free fluid in pelvis, but that will usually presents as surgical emergency,
sometimes in very early stages the sac can get absorbed giving above picture,
usually ectopic pregnancy will be diagnosed after around 5 weeks of last menstrual period means around 2 to 3 weeks after ovulation or intercourse,
so what you can do is go for blood test for pregnancy which will detect the elevated levels of HCG in case of positivity and can also suggest ectopic pregnancy.
